Comments:
An unannounced monitoring inspection was conducted on 5/24/2024. There were 14 children, ages 3 years-5 years and 3 staff present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, care of children, preventing the spread of disease, emergencies and nutrition. A total of 3 children?s records and 1 staff record were reviewed. The children were observed during circle time and center time. The inspector arrived for the inspection at 10:40am and departed the center at 11:20am.

Information gathered during the inspection determined compliance with applicable standards or law. No violations were issued.

Violations:
Standard #: 8VAC20-780-60-A
Description: Based on record review and discussion with staff, the center failed to ensure that the separate record for each child enrolled contains all of the elements as required by the standards.

Evidence: The record for Child 1 did not contain the following information: name of the child?s physician.

Plan of Correction: Provider has obtained physicians name.
