63.2(17) License & Registration Procedures

Comments:
An unannounced monitoring inspection was conducted on July 16, 2019 from approximately 1:35pm-4:35pm. Upon arrival to the facility, there were 134 children with 15 staff members ranging in age from infant to school age. The children were observed resting, doing an arts activity, playing in centers and talking and playing freely amongst themselves. The center does offer transportation.

Violations:
Standard #: 22VAC40-191-40-D-2
Description: Based on record review, in one of 13 staff records reviewed, the center did not ensure that staff have results of a search of the central registry within 30 days of employment. Evidence: The Director confirmed that there was no documentation of a Central Registry finding for Staff #9 (hire date 10/16/18).

Plan of Correction: We will have the form notarized and the paperwork will be sent off tomorrow.

Standard #: 63.2(17)-1716-A-4
Description: Based on review, in one of 13 staff records reviewed, the center did not ensure that each person in a supervisory position has been certified by a practicing physician or physician assistant to be free from any disability which would prevent him from caring for children under his supervision. Evidence: As confirmed by the Director, there was no documentation that Staff #13 had been certified by a practicing physician or physician assistant to be free from any disability which would prevent her from caring for children under her supervision.

Plan of Correction: Director will ask the staff member to make sure that she gets it in.

Standard #: 63.2(17)-1716-A-6
Description: Based on review, the center did not ensure all required information was described in a written statement to the parents or guardians of the children in the center and made available to the general public. Evidence: The Director confirmed that the center had not disclosed in a written statement whether or not the facility had public liability insurence.

Plan of Correction: We are currently updating the parent handbook and will add that information for the upcoming school year.

Standard #: 63.2(17)-1716-B-3
Description: Based on observation and inspection of the facility, the center had not established and implemented procedures to ensure that there was a person trained to perform a daily simple health screening. Evidence: The Director confirmed that there was not established procedures to ensure that there was a person trained to perform a daily simple health screening.

Plan of Correction: Director will make sure that she or the Assistant Director completes the training. A staff meeting has been scheduled for August 6, 2019 and the training will be given to the staff at that time.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on record review, in two of 13 staff records reviewed, the center did not obtain a copy of the results of a search of the central registry maintained pursuant to ? 63.2-1515 and any child abuse and neglect registry or equivalent registry maintained by any other state in which the individual has resided in the preceding five years. Evidence: The Director confirmed that there was no documentation that the center had conducted out of states child abuse and neglect searches for the following staff members who indicated on their sworn statements that they lived outside of Virginia within the last five years: Staff #9 (date of hire 10/16/18) Staff #13- (date of hire 4/26/19)

Plan of Correction: Director will have the forms printed and signed tomorrow.
