Technical Assistance:
Hazardous substances such as cleaning materials, insecticides, and pesticides shall be
kept in a locked place using a safe locking method that prevents access by children. If a
key is used, the key shall not be accessible to children. Cleaning supplies to clean and
sanitize the diapering area or toilet chairs do not need to be kept locked during diapering
or toilet training time as long as they are inaccessible to children.

Comments:
An unannounced subsidy health and safety inspection was initiated on 6/2/2022 and concluded on 6/2/2022 from 9:55 AM to 12:50 PM. There were 78 children present, ranging in ages from infant to 12, with 20 staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, procedures and medication. A total of three children?s records and three staff?s records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 8VAC20-790-550-2-a
Description: Based on record review and interview, the center failed to obtain a central registry background check within 30 days after employment.

Evidence:

1. The record for staff 3 documents the start date as 12/7/21. There are no results for the
central registry background check for staff 3 in the staff record.
2. The director verified the central registry background check was mailed within seven days of hire but no results were received. They did not follow-up to see why the results had not been received.

Plan of Correction: During the inspection the director contacted the office of background investigations (OBI) by email to find out the status of the background check.
Licensing was cc'ed on the email. The director will find out if the check has been cashed. If OBI does not respond in a week, a new central registry background check (CPS) will be completed for staff 2. Licensing will be notified of the progress.
In the future CPS background checks will be mailed within seven days of hire and if the results are not received within 30 days OBI will be contacted. The record will be documented of the date of mailing and the date of follow-up.

Standard #: 8VAC20-790-820-E
Description: Based on observation, the center failed to label prepared infant formula with the date and the child's name.

Evidence:

In the infant room, four infant bottles were found in the refrigerated with no date or name on the bottle.

Plan of Correction: The bottles were labeled during the inspection. Staff will be reminded to check bottles when parents arrive in the mornings with bottles to make sure they are labeled with dates and names.
