22.1 BACKGROUND CHECKS CODE; CARBON MONOXIDE

Comments:
An unannounced monitoring inspection was initiated on June 10, 2024 and concluded on June 14, 2024. Eight staff were present supervising forty-one children from ages infants through school age. Children were observed eating lunch, washing their hands and napping. Five staff and five child records were reviewed. Violations found and are noted on the violation notice.

Violations:
Standard #: 8VAC20-780-160-C
Description: Based on review, facility did not ensure staff complete an updated TB test or screening every two years.

Evidence: The record for staff #4 had a most recent TB test/screening dated 2/18/2022 which is more than two years old.

Plan of Correction: Staff will have TB test/screening done.

Standard #: 8VAC20-780-80-A
Description: Based on observation and review, facility did not ensure there is a written record of daily attendance that documents arrival and departure of each child in care as it occurs for each group of children.

Evidence: The following classrooms did not have an attendance documentation for their group of children: school age, 2's, 3's and 4/5's. The attendance for these classrooms were combined with other groups.

Plan of Correction: Staff will start using the original attendance sheets to ensure attendance is clearly documented for each group.

Standard #: 8VAC20-780-330-B
Description: Repeat Violation
Based on observation, facility did not ensure there is adequate resilient surfacing under and around climbing parts or equipment with moving parts on the outside playground.

Evidence: The following was observed on the playground:
1. On the toddler playground, a 14" high plastic slide was stored on dirt directly against the fence. There was no resilient surfacing and no fall zone.
2. On the big playground, a 3 seat merry-go-round and tricycle merry-go-round were stored on dirt with no resilient surfacing.
3. On the big playground, a 4 seat teeter-totter and arch climber had less than 1" of resilient surfacing around the exterior when there should be at least 6" depth.
4. On the big playground, there was approximately 4' of fall zone between the arch climber and the 3 seater steering bench when there should be at least 6' of fall zone.
5. On the big playground, a 17" high blue airplane rocker had approximately 1" of resilient surfacing when it should have at least 6" depth.

Plan of Correction: The plastic slide will be removed from the toddler playground today. Mulch will be raked into areas on the playground that have climbing and/or moving parts. An estimate is being obtained within a month for the playground to be re-done by a certified playground company.
