Standard #: 22VAC40-111-90-A Description: Based on record review, the provider did not ensure before a child may attend the family day home, the provider shall obtain documentation that the child has been adequately immunized.
Evidence: The record of Child #1 (DOE 10/5/20) contained documentation of immunizations dated 10/9/20.Plan of Correction: I will ensure that no child going forth will be allowed to start attending daycare until physical and immunization records are completed and received before start date, as well as all other required documents. Put planner in place fore reminders of checklist.
