Technical Assistance:
Effective January 1, 2025, the VDOE will begin determining compliance with ? 22.1-289.057 of the Code of Virginia, which is legislation passed by the General Assembly in 2020. The law requires all licensed child day programs, religious exempt child day centers that serve preschool age children, and certified preschools to test potable drinking water. The law requires that programs submit their plans and test results to the Virginia Department of Health Office of Drinking Water (VDH ODW) and the Superintendent. If the results of the testing indicate elevated lead levels, the program shall remediate, retest, and resubmit results to VDH ODW and the Superintendent. There is an additional alternative bottled water option that comes with additional requirements. Comments:
A monitoring inspection was conducted May 22, 2024. Two licensing inspectors were onsite, completing the inspection from approximately 1:00pm until 4:00pm. There were 22 children present, ranging in ages from two to 11 years, with five staff supervising. The inspectors reviewed compliance in the areas of administration, physical plant, staffing and supervision, and medication. A total of five children's records and five staff members? records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice, sign, and return it to me within five business days from the date of receipt. You should specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the stand(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Description: REPEAT VIOLATION:
Based on observations and interview, the center did not maintain all areas and equipment in a clean, safe, and operable condition.
Evidence:
The outdoor playground contained water hoses and outdoor carpet that were curling up, creating a tripping hazard for the children.

Description: Based on observations, the center did not ensure all hazardous substances such as cleaning materials, were kept in a locked place using a safe locking method that prevented access by children.
Evidence:
1. Unlocked sanitizing wipes were located in the Preschool classroom.
2. The kitchen door was propped open. The kitchen contained unlocked cleaning chemicals. The kitchen door opens directly in the classroom. Children were observed in the classroom.
3. The cleaning chemicals were labeled Danger and Caution: Keep out of the reach of children.

Standard #: 8VAC20-780-330-B
Description: Based on observation and measurements, the center did not ensure areas where play equipment is provided, resilient surfacing complied with minimum safety standards described in the American Society for Testing and Materials.

Evidence:
1. A piece of climbing equipment, approximately 42 inches in height, was located on playground. The depth of wood chips underneath the play equipment measured approximately three to four inches in depth at varying locations within the play equipment?s fall zones.
2.The height of the climbing equipment required at least six inches in depth of wood chips in all fall zone areas.
