The physical condition of the van was discussed.

Comments:
A complaint inspection was conducted on 6/7/24 in response to a complaint. The complaint was related to the transportation of children to the center from their respective elementary schools on 5/14/24. Two staff were interviewed and the center's van logs were reviewed. The evidence gathered during the investigation did not support the allegations of non-compliance with standards or law. Description: Based on observation, areas and equipment outside the center was not maintained in a clean, safe and operable condition.
Evidence:
1. One of the three vans that are used by the center for the transportation of children was not maintained in a clean, safe and operable condition.
2. The carpeting on the floor and the seats of the van were visibly dirty with stains, leftover food and dirt under and on the seats. This was observed from the front of the van to the back.
3. The cloth for the second last seat was torn and pulled down, exposing the torn and broken foam underneath. The metal frame of the seat was exposed as the foam had moved from it's position, posing an injury hazard for children.
4. A small compartment on the left side of the van under the window was broken, exposing a jagged and sharp plastic edge posing an injury hazard to the children.
5. The booster seats in the van appeared to be stained and at least one of them did not have the seat cover.
