63.2(17) License & Registration Procedures

Comments:
A Unannounced Mandated Monitoring inspection was conducted on December 3, 2019 from approximately 12:45 pm to 4:35pm. Upon arrival to the facility, there were 99 preschool with thirteen staff members. Additional staff were on site for administrative and support purposes. The children were observed in a variety of activities included but not limited to napping, story time, singing, playing outside and preparing for the holidays. Six medications were reviewed. Eleven children's records and six staff records were reviewed.

Violations:
Standard #: 22VAC40-185-260-B
Description: Based on record review and staff interview, the licensee did not ensure there was annual health inspection.

Evidence:
The most recent health inspection available was from June 26, 2018. The health inspection was more than a year old.

Plan of Correction: The Director will check with the Building Operations Director on a bi-Monthly basis to ensure all inspections needed are schedule and up to date. To resolve this violation, Our Building Operations Director contacted the Health Inspector who came out to the facility and completed an inspection on the 4th of December. Please see the inspection report attached

Standard #: 22VAC40-185-280-B
Description: Based on observation and inspection of the facility, the licensee did not ensure that hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children. If a key is used, the key shall not be accessible to the children.

Evidence: In preschool classroom #6, there was three bottles of hand sanitizer in the classroom sitting on the cubbies and desk. The hand sanitizer label was "warning" and "keep out of reach of children."

Plan of Correction: ? The Director will check with the Building Operations Director on a bi-Monthly basis to ensure all inspections needed are schedule and up to date. To resolve this violation, Our Building Operations Director contacted the Health Inspector who came out to the facility and completed an inspection on the 4th of December. Please see the inspection report attached.

The Director will hold a staff meeting and remind all teachers that all Hazardous substances must be kept in a locked cabinet. The assistant director will visit each classroom to ensure that this has been followed through on. The hand sanitizer that was out during the inspection, was put in a locked cabinet as soon as it was pointed out.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on review and interview, facility did not ensure there is a search of the child abuse and neglect registry or equivalent maintained by any other state when an employee has lived outside the state of Virginia within the past 5 years.

Evidence: The record for staff #4 was reviewed. According the sworn statement or affirmation completed on August 27, 2019, the individual lived outside of the state VA in the past five years. There is not a completed child abuse and neglect registry or documentation of the equivalent for the state in which the individual resided.

Plan of Correction: The Director of the program will keep any sworn statements that require out of state background checks and will submit the request for a search. They will make a copy of the search request and date it when it is sent in. Once the results are returned, they will place the results along with the Sworn Statement in the staff file.
I have had my staff fill out another request for the state of South Carolina and will be sending it in while I wait to hear back about the status of the previous request. (See new request attached).
