Comments:

An unannounced renewal inspection was conducted on June 3, 202. There were 48 children present, ranging in ages from 3 years to 5 years, with 9 staff supervising. The inspection began at 11:30 AM and concluded at 3:00 PM. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records, 7 staff records, and 4 board member records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Description:	(Repeat Violation) Based on record review and interview, the center did not ensure that all staff records were complete. Evidence: 1. The record for staff #3 did not contain education, orientation, or documentation that two or more references as to character and reputation as well as competency were checked before employment or volunteering. Staff #3?s hire date was 1/24/2024. 2. The record for staff #4 did not contain orientation. Staff #4?s hire date was 2/14/2024. 3. The record for staff #5 did not contain orientation. Description:	(Repeat Violation) Based on record review and interview, it was determined that the center did not ensure that they maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence.
