An unannounced monitoring inspection was conducted on 6/10/2024 between the hours of 7:30am-9:00am. The physical plant, 3 staff records, 4 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed playing outside. Areas of non-compliance are identified in the Violation Notice.

Standard #: 8VAC20-780-140-A Description: Based on record review, the center did not ensure 1 of 4 children has received a physical exam within 30 days of the child's first date of attendance.
Evidence:
1. The record for Child #4 (start date 8/22/2022) did not contain documentation of a physical examination.Plan of Correction: The center will request the physical.
Standard #: 8VAC20-780-60-A-8 Description: Based on record review, the center did not obtain a written care plan for one child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

1. Child #2 is diagnosed with a food allergy.
2. The record for Child #2 did not contain documentation of a written care plan with instructions from a physician regarding the food allergy.Plan of Correction: Will request an allergy care plan for the child.
