Evidence:
1. The record of staff #2 (DOH:05/01/2017) contained documentation of the Virginia Annual Health and Safety Update dated 05/27/2020.
2. The vendor acknowledged that the training update was not completed by 03/01/2020 as required.Plan of Correction: I will make sure all staff complete annually the virginia annual health and safety course sponsored by the Virginia department of social services as required on time.
