A renewal was conducted on 04/25/1014. A tour of the home was conducted, in addition to a record review, interviews with staff and requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Description:	Based on a record review the licensee did not ensure all required information was in each child's record. Evidence: Child #4 did not have documentation of one of the two required emergency contacts.
Plan of Correction:	The provider will obtain the child's information at pick-up today.
