Evidence:
1. There was no documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance for Child #1 and Child #2.
2. In the children's records, Child #1's start date is listed as 1/4/2024 and Child #2's start date is listed as 12/28/2023.Plan of Correction: I will ask the parents today.
Standard #: 8VAC20-800-320-A Description: Based on observation, small electrical appliances were not unplugged when not being used.
Evidence: A toaster oven was observed plugged in when not in use.Plan of Correction: Corrected during inspection. I will let the assistants know for the future.
Standard #: 8VAC20-800-430-B Description: Based on observation and interview, the provider did not ensure accordion expansion gates and pressure-mounted gates were not used as protective barriers at stair openings.
Evidence: A pressure-mounted gate was observed at the bottom of the stairway opening leading to the second floor.Plan of Correction: I will install as soon as I can.
