An unannounced monitoring inspection was conducted today from 9:00am-10:00am. The home, 3 caregiver records, 4 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed during storytime and playing freely.

The most recent sworn statement or affirmation in the record for Household Member #1 was dated 11/01/2018.Plan of Correction: Provider will obtain updated background checks for the household member.
