Comments:
An unannounced monitoring visit and facility tour was conducted on 04/15/2024. At the time of inspectors' arrival there were 20 infant to preschool aged children in care with 5 staff members and 1administrator. Children were observed interacting with staff, and engaging in free play, educational activities, and hand washing. A sample of 5 children's records and 5 staff records were reviewed. Injury reports and medications were also reviewed. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.

Standard #: 22.1-289.035-A
Description: Based on record review and interview it was determined that the center did not ensure each employee completes repeat background checks every five years.
Evidence:
1. The records for staff #1, #2, #3, and #4 did not contain documentation of repeat criminal background checks.
2. The documentation available was dated 11/29/18, 11/20/18, 11/27/18, and 11/27/18 respectively.
3. Staff confirmed that they had not performed repeat background checks.

Standard #: 8VAC20-780-160-C
Description: Based on record review and interview it was
determined that center did not ensure that staff obtain follow up tuberculosis screenings at least
every two years.
Evidence:
1. The record for staff #3 did not contain documentation of repeat tuberculosis.
2. The documentation available was dated 4/30/21.
3. Staff confirmed that they had not performed repeat tuberculosis screenings.

Standard #: 8VAC20-780-270-A
Description: Based on observation and interview it was determined the center did not ensure that all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition.
Evidence:
1. In the Pooh and Friends classroom there was peeling paint on the window sills and one approximately 2'x2.5' inner glass window pane with a crack spanning the length of the window leaving an exposed sharp glass edge accessible by the children in care.
2.There was splintered plywood on the front of the sink, rust on the boys bathroom partition, and stained, sagging ceiling tiles, wet blankets and buckets catching water in the Sesame Street classroom.
3. Outside on the playground there was a tricycle with no pedals and exposed rust on the rocker.

Description: Based on observation it was determined that the center did not ensure that hazardous substances were kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. There wee unlocked chemicals in the Rugrats room under the changing table.
2. The electrical room was unlocked, chemicals were stored in this area.
