Standard #: 8VAC20-780-40-E Description: Based on a review of policies and procedures, interviews, and a review of documentation, the center did not ensure that the center's activities and services are maintained in compliance with the center's own policies and procedures required by the standards.
Evidence:
1. The facility's Accident and Injury policy states: "If your child has a minor accident resulting in injury, our teachers will administer the necessary first aid care such as band-aids and ice packs and will complete an Accident Report form detailing the incident. This form will be given to you at the end of the day. Please sign it and return it to the Director for our records."
2. Child #1 was observed to have scratches on face. An accident report was not available for review at the time of the investigation.
3. A member of management states that the accident report could not be located.Plan of Correction: "Written steps to complete an Accident/Injury report has been added to the Staff Manual."
Standard #: 8VAC20-780-550-P Description: Based on review and an interview, the center did not ensure to maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence.
Evidence:
1.The record for child #1 did not contain a record of an injury.
2. A member of management was unable to locate an accident report for child #1.Plan of Correction: "Written steps to complete an Accident/Injury report has been added to the Staff Manual."
