Comments:
An unannounced monitoring inspection was conducted on 5/6/24 from 11:30 a.m. to 12:30 p.m. with the provider. There were three children in care, ranging in age from 16 months to three-years-old, for a total of seven points. Children were observed eating breakfast. Five child records, one staff record, and one household member record were reviewed. The provider has current certification in CPR and First Aid. No medications were on site and the provider understands that they must obtain certification in Medication Administration Training (MAT) in order to give medication. The first aid kit, flashlight, weather band radio, and fire extinguisher were observed. The emergency drill log, attendance, and menu were also reviewed. Areas of non-compliance are identified in the Violation Notice. Standard #: 8VAC20-800-780-B
Description: Based on review of five child records, the provider did not ensure that they reviewed with the parent the emergency contact information annually for each child and obtained the parent's signature acknowledging the review.

Evidence: The record for Child #5, with a start date of 9/2/20, did not contain documentation of an annual review with parent signature.

Standard #: 8VAC20-800-980-A
Description: Based on review of five child records and interview, not all children's records contain general written permission from the parent of each child for the provider to take the child off the premises of the family day home.

Evidence: The provider did not obtain written permission from the parent of Child #1 to take the child off the premises of the family day home to pick up and drop off children that attend public school.

Description: Based on review of five child records, the provider did not maintain an up-to-date record at the family day home for each enrolled child.
