Standard #: 22VAC40-111-240-A Description: Based on observation, outside equipment of the family day home was not maintained in a safe condition.
Evidence: The round slide on the playground had pieces that were broken and coming loose, creating a gap between two parts of the slide that could pose an entrapment hazard.Plan of Correction: The children are not using the slide. I will have someone repair the slide and until then the children will not be allowed to use the slide.
