Discussed testing water for lead.

Comments:
A monitoring inspection was initiated on 6/12/2024 at approximately 10:30am and concluded on 6/12/2024 at approximately 12:50pm. There were a total of 11children (25 points) observed with 3 caregivers present. The children were observed playing at a nearby park, washing hands, and eating lunch. The inspector reviewed compliance in the areas of administration, personnel and household members, physical environment, care of children, handwashing, diapering, medication, emergencies and nutrition. A total of 10 child records and 7 caregiver/household member records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Description: Based on review of staff and household records, criminal history record checks were not completed every 5 years.

Evidence: Criminal history record checks were not completed every 5 years for Staff #2, and Household Members #1, #2, #3, and #4.

Plan of Correction: Provider will obtain updated Criminal History record checks for staff and household members.

Description: Based on review of assistant and household files, sworn statements were not updated every 5 years.

Evidence: Sworn statements were not updated every 5 years for Staff #1 and Household Member #3 and #4.

Evidence: There was no documentation that the Provider annually reviewed with the parent the emergency contact information to ensure the information is correct for child #1,#4, #7, #8, and #9.

Plan of Correction: Provider will obtain documentation of annual updates of emergency contact information for the children.

Description: Based on review of children's records, the Provider did not obtain documentation of a physical examination within 30 days after the first day of attendance.

Evidence: There was no physical for Child #3, #4, #6, and #9.

Plan of Correction: Provider will obtain record of physical exams for the children.

Standard #: 8VAC20-800-60-B
Description: Based on review of children's files, the provider did not obtain the name, address, and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached.

Evidence: There were not two designated persons to contact in case of an emergency if the parent cannot be reached for Child #2, #4, #5, #7, #9.

Plan of Correction: Provider will obtain two emergency contacts for all children.
