Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

An unannounced code compliance inspection was initiated on 2/23/21 and concluded on 12/24/21. The person in-charge was contacted by telephone to initiate the inspection. There were 8 children present and 1 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-191-40-D-2
Description: Based on record review and interview, the center failed to have staff sign a sworn disclosure statement prior to the first day of employment and have a completed central registry record check by the end of the 30th day of employment.

Evidence:

1. The record for staff 1 documents the start date as 7/31/19. The sworn statement is dated 8/16/19 & the central registry findings are dated 11/30/20.
2. The administrator verified staff 1 hire date for the exempt program was 7/31/19.

Plan of Correction: In the future the administrator will ensure the sworn statement is completed prior to the first day of employment and the central registry record check is mailed upon hire and followed up if the results are not received within 30 days.

Description: Based on record review and interview, the center failed to obtain fingerprint-based criminal history check determination letters prior to the first day of employment.

Evidence:

1. The record for staff 1 documents the hire date as 7/31/19. The fingerprint-based criminal history check determination letter is dated 8/12/19.
2. The administrator verified staff 1 hire date for the exempt program was 7/31/19.

Plan of Correction: In the future the administrator will ensure new staff will not be hired until the fingerprint-based criminal history check determination letter is received.
