Comments:
An unannounced monitoring inspection was conducted on 7/10/2024 from 10:40 a.m.-12:00 p.m. There were 4 children in care, ranging in age from 2 years to 5 years with one caregiver supervising. The inspector reviewed compliance in the areas of administration, personnel, household members, physical environment and equipment, care of children, emergencies, caregiver training, preventing the spread of disease and nutrition. A total of four children?s records and one caregiver record were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or laws and violations were documented on the violation notice issued to the program.

Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it within 5 business days. Please specify how the deficient practice will be or has been corrected.

Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Standard #: 8VAC20-800-770
Description: Based on observation and interview, the provider did not have extra batteries available to use in her weather band radio in the event of an emergency.

Evidence: Provider #1 had a weather band radio in the home, but the batteries were old, and it was not working. Provider #1 acknowledged that she needed to obtain extra batteries for the radio.

Standard #: 8VAC20-800-790
Description: Based on observation and interview, the provider did not have emergency numbers for police, fire, emergency medical responders, a person responsible for emergency backup care, or the regional poison control center posted close to the telephone.

Evidence: No posting of emergency numbers was observed in the home. Provider #1 acknowledged that she did not have the required emergency numbers posted in a visible area close to the telephone.

Standard #: 8VAC20-800-70-A
Description: Based on review of four children?s records and interview, the provider did not have documentation that the parents were provided with written information regarding the requirement to pick up their child if necessary, when they are sick and to inform the provider within 24 hours if their child develops a communicable disease before the child?s first date of attendance.

Evidence: There was no documentation that this written information was provided to the parents in the children?s files. The four files reviewed were comprised of children in attendance at the time of the inspection. Provider #1 acknowledged that she needed to provide that written information to the parents.

Standard #: 8VAC20-800-180-A
Description: Based on review of one caregiver?s record and interview, the caregiver did not obtain a current report of tuberculosis screening every two years from the date of the first screening as required.

Evidence: The most recent Report of Tuberculosis (TB) Screening for Provider #1 was dated 5/08/22. Provider #1 acknowledged that she had not obtained new TB test results.

Standard #: 8VAC20-800-330-A
Description: Based on observation and interview, the caregiver did not have a landline telephone available, operable, or accessible during the family day home?s hours of operation.

Evidence: There was no landline phone observed in the home. Provider #1 acknowledged that she did not have a landline telephone as required.
