Comments:
An unannounced monitoring inspection was conducted on May 16th, 2024. The licensing inspector was on site from approximately 9:56am until 11:15am. There were no children present with one caregiver present, totaling zero points. Toys, materials & spaces were viewed for all ages and in safe conditions.
The inspectors reviewed compliance in the areas of administration, personnel, household members, caregiver training, physical equipment and environment, emergencies, nutrition, medication administration and care of children. A total of two child records and two caregiver records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1)
steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). If you have any questions, please contact the licensing inspector.

Standard #: 8VAC20-800-180-A
Description: Based on caregiver record review, the provider did not ensure to maintain a current report of Tuberculosis Screening for each caregiver and adult household members.

Evidence:
1. Caregiver #1 did not have documentation of a current Tuberculosis screening report. The last updated TB screening was dated 5/3/2022.
2. Caregiver #1 mentioned that the TB screening was completed, but could not provide documentation of the results.
