Comments:
A monitoring inspection was conducted on 05/22/2024. There were no children present with 2 caregivers. The children were engaged in the following: toy play, games, movie time, snack, bathroom breaks, hand washing, and other organized activities. The inspector reviewed compliance in the areas of administration, personnel and household members, physical environment, care of children, handwashing, diapering, medication, emergencies and nutrition. A total of 1 child records and 2 caregiver/household member records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today in order for it to be included on the website. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Description: Based on review of records, the provider did not ensure all required background checks were current.

Evidence: Staff#2 Fingerprints were expired.

Plan of Correction: We will submit the background check.

Standard #: 8VAC20-800-830-A
Description: REPEAT VIOLATION
Based on a review of documentation, the emergency evacuation procedures were not practiced monthly.
Evidence:
The last drill was on 11/15/2023.

Plan of Correction: I will practice and document the drills.

Standard #: 8VAC20-800-920
Description: Based on a review of documentation, the provider did not ensure all menu requirements were met.
Evidence:
The menu was not dated for the one week period.

Plan of Correction: I will date the menu.

Standard #: 8VAC20-800-240-A
Description: Based on observation, the provider did not ensure areas of the family day home were maintained in a clean, safe, and operable condition.
Evidence:
There was chipped paint observed approximately 3 t0 4 inches in length, on the right wall between the staircase and bathroom and on the left wall at the bottom of the staircase approximately 1 to 2 inches in length.

Plan of Correction: I will put up a barrier or paint and patch the wall.
