Technical Assistance:
**Discussed with the director to ensure that all foods that can potentially be a choking hazards must be cut for children 3 years of age and younger. **Discussed with the director that when volunteers are used for ratios and supervise children on their own, they are considered staff at that time and will need complete staff records. **Please ensure that you send me documentation of your DHO training as soon as it is received.

Comments:
An unannounced Renewal Inspection was conducted today from 11:29am to 2:00pm. There were 20 (22 months to 4 years of age) directly supervised by 5 staff. The physical plant, 5 staff records, 5 children?s records, 2 children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, listening to stories, eating lunch, and preparing for and taking naps. Hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid. Staff-to-child ratios were not in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Standard #: 22VAC40-185-40-J
Description: Based on review, the facility failed to ensure that the injury prevention procedures were updated at least annually based on documentation of injuries and a review of the activities and services. Evidence: identified through the director that there was no documentation of an annually updated injury prevention plan.

Plan of Correction: Will complete within a week.

Standard #: 22VAC40-185-60-A
Description: Based on review, the facility failed to ensure that children's records were maintained with the required information. Evidence: 1) Child 1, 2, 4 and 5 did not have documentation of complete emergency contact information. 2) Child 1, 3 and 5 did not have documentation of the parents review of emergency contacts within the last 12 months.

Plan of Correction: Will be corrected in two weeks.

Standard #: 22VAC40-185-70-A
Description: Based on review, the facility failed to ensure that staff records were maintained with the required documentation. Evidence: Reviewed 5 staff records and found that: 1) Staff 1 only had 1 documented reference check and Staff 5 did not have any documentation of reference checks. 2) Staff 5 did not have documentation of an emergency contact 3) Staff 5 did not have documentation of an application.

Plan of Correction: I will get the reference check for Staff 1 today and Staff 5 will no longer be used for maintaining ratios and will be a volunteer only.

Description: Based on review, the facility failed to ensure that the program leader has documentation of qualifications. Evidence: Reviewed 5 staff records and found that Staff 1 and Staff 5 did not have documentation of program leader qualifications.

Plan of Correction: Will ask staff for documentation today.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the facility failed to ensure that hazardous substances are kept in a locked place using a safe locking method that prevents access by children. Evidence: Observed in the Pre Kindergarten classroom, a bottle of Febreez that was in a bag on a shelf accessible to children.

Plan of Correction: Corrected at the time of the visit.

Standard #: 22VAC40-185-350-C
Description: Based on observation, the facility failed to ensure that when children are regularly in ongoing mixed age groups, the staff to child ratio applicable to the youngest child in the group shall apply to the entire group. Evidence: Observed in the 2 year old classroom: 6 children and 1 staff member - 5 that were 2 years of age and 1 that was 22 months old. The ratio for the youngest child is 1:5.

Standard #: 22VAC40-185-430-B
Description: Based on observation, the facility failed to ensure that materials and equipment available shall be age and stage appropriate for the children in care. Evidence: Observed in the Pre-K room, a large trash bag on the door handle with another empty trash bag inside and accessible to children.

Plan of Correction: Corrected at the time of the visit.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the facility failed to ensure that the diapers are disposed of in a system that is not hand operated. Evidence: the diaper disposal was not working properly, creating a system where the staff member needs to open the can with their hand to properly throw items away.

Plan of Correction: Will fix or replace the the disposal system.

Standard #: 22VAC40-185-550-M
Description: Based on review, the facility failed to ensure that the written injury records were maintained with the required information. Evidence: Reviewed 10 injury records in children's records and in the injury folder and found that 3 did not have documentation of the time parents were notified of the injuries.

Plan of Correction: Will remind staff to complete injury reports.

Standard #: 22VAC40-185-560-F-4
Description: Based on observation, the facility failed to ensure that children three years of age and younger are not offered foods that are considered to be potential choking hazards. Evidence: Observed in the Preschool (3 year olds) and two year old classroom (including one child that was 22 months old) were being served grapes with their lunch that were not cut to prevent them from becoming a choking hazard. Identified through staff that they do not cut the grapes that are given to them whole from the cafeteria.

Plan of Correction: Will notify the kitchen that grapes must be cut to prevent choking hazards.

Standard #: 22VAC40-185-560-G
Description: Based on observation, the facility failed to ensure that food brought from home is clearly labeled with the owner and the date. Evidence: Observed in the preschool classroom a lunch box without a name or date.

Plan of Correction: It will be labeled today.
