Description: Based on interviews the center failed to ensure that children under 10 years of age
always shall be within actual sight and sound supervision of staff.

S2 reported that she and another staff were supervising the toddlers and 2-year-olds in the center?s inside playground. She stepped away briefly from the group to attend to another child needs. S2 did not realize that C1 had gone into the classroom out of actual sight and supervision of staff until the child?s mother came in to pick up the child and located the child.
