Comments:
An unannounced monitoring inspection was conducted on 05/08/2024 from 9:30am to 10:40am with the Provider. There were 11 children ages 8 months to age 4 years old (27 points) with the Provider and 2 assistants. The programming area, attendance, outdoor playground area, menus, Provider, Assistant and Household member records, 3 children?s records, emergency drills, emergency supplies, and policies were reviewed. Children were observed participating in napping and art. Handwashing procedures were also observed. There was an adequate number of staff present with current certification in CPR and First Aid training. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 8VAC20-800-920
Description: Based on documentation and provider interview, the provider did not have a documentation of a menu for the current one-week period or a menu kept on file for one week.

Evidence: During the inspection, the provider stated that they did not have a menu ready for the remainder of the current week.

Plan of Correction: Menu was done same day of inspection.

Standard #: 8VAC20-800-50-A
Description: Based on review of the written attendance record and interview with the Provider, the Provider did not ensure they maintained a written record of children in attendance each day.

Evidence:
During the inspection in was determined there were 10 children present, but the written record reflected there were 8 children present.

Plan of Correction: Was written down (2) names while doing inspection.

Standard #: 8VAC20-800-320-G
Description: Based on review of documentation and Provider interview, the Provider did not ensure that fireplaces and associated chimneys were inspected annually and cleaned as needed.

Evidence: The Provider had documentation that the last fireplace inspection was completed on 1/13/23 and stated that it had not been inspected since this date.

Plan of Correction: Will be inspected by the end of May.

Standard #: 8VAC20-800-500-D
Description: Based on observation, the Provider did not ensure that outdoor play equipment did not have equipment that may pinch or crush children?s hands or fingers.

Evidence: The brown plastic play structure had large cracks in the plastic on 2 sides that could pinch a child?s hand or fingers.

Plan of Correction: Was fixed same day of inspection.

Standard #: 8VAC20-800-690-D
Description: Based on observation, the Provider did not ensure that the diapering surface was cleaned and sanitized after use.

Evidence: During inspection, residue and debris were noticed on the diaper pad surface.

Plan of Correction: Was cleaned while doing inspection.
