Comments:
A mandated monitoring inspection was conducted on December 12, 2023 on site from 10:00 A.M.-11:50 P.M., and concluded on December 15, 2023 after a desk review of staff records. There were 27 children present, ranging in ages three to five years of age from, with six staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of six child records and five staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-770-60-B
Description: Based on a review of staff records, the center failed to ensure that a sworn disclosure statement was completed prior to the start of employment.

Evidence:
1. Staff #2's date of hire was 10/18/2021. The sworn disclosure statement in the file was dated 10/26/2021.
2.Administrative staff verified the date of employment and the date on the sworn disclosure statement.

Plan of Correction: New employees will complete a sworn disclosure state prior to employment moving forward.

Standard #: 8VAC20-780-270-A
Description: Based on observation, the center failed to ensure that all equipment and materials inside shall be maintained in a clean, safe, and operable condition.

Evidence:
In Classroom #2 the blinds on the back door are broken and bent. One of the slats of the blinds is bent out causing a hazard.

Plan of Correction: Broken piece will be removed and new blinds will be installed.

Standard #: 8VAC20-780-330-B
Description: Based on observation of the playground area, the center failed to ensure that all areas that require resilient surfacing had the required amount.

Evidence:
The mulch that is at the bottom of the yellow slides and the fall zones surrounding the equipment is disintegrated and packed down with bare ground showing through.

Plan of Correction: Mulch will be fluffed in effected areas, and new mulch will be laid in January 2024.
