Comments:
An unannounced monitoring inspection was conducted on 05/14/2024. There were 13 children, ages 2 years- 5 years, 3 staff members, and 1 volunteer present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, physical plant, programs, emergencies and nutrition. The children were observed in free choice activities, participating in learning activities, and during music time. A total of 5 children?s records and 4 staff records were reviewed. The inspector discussed resilient surfacing for playgrounds with the provider. The inspector arrived for the inspection at 9:15 am and departed at 11:30 am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Violations:
Standard #: 8VAC20-780-580-C
Description: Based on observations and staff interviews, the center failed to ensure that during transportation of children the Virginia state statutes about safety belts and child restraints are followed as required by the Code of Virginia and that the information required by the standard is in the transportation vehicle.

Evidence: Staff 2 stated that a total of 7 children, 4-5 on a daily basis, are transported by the center for after school care via the white van. Review of birth dates for the 7 children determined that 4 of the children are under the age of 8 years. There was only one child restraint device observed in the white van that is used daily for transportation. The Code of Virginia requires that any child, up to age eight, shall be properly secured in a child restraint device of a type which meets the standards adopted by the United States Department of Transportation.

Plan of Correction: Will purchase booster seats.
