Comments:
An unannounced monitoring inspection was conducted today from 9:15am to 12:30pm with the Assistant Director. There were 49 children in care, ranging from five months to twelve years of age, supervised by six staff. The children were observed having free play, washing hands for snack, eating snack, and during circle time. Six children?s records and five staff records were reviewed and were in compliance. All of the center staff have current certification in CPR and First Aid, as well as Daily Health Observation training. Two medications and authorizations were reviewed and the center has three staff current in Medication Administration Training (MAT). The attendance and emergency drill log were reviewed. The most recent Fire Inspection on file was dated 7/17/19 and the most recent Health Inspection on file was dated 11/18/19. The most recent documented evacuation drill was dated 12/4/19 and the most recent documented shelter-in-place drill was dated 9/27/19. Morning snack today was goldfish crackers and apple or grape juice. Violations:
Standard #: 22VAC40-185-80-A
Description: Based on documentation review and interview with staff, the center did not keep a written record of children in attendance. Evidence: 1. In the Infant class there were three children present but two children were documented as present on the attendance record. 2. In the two-year-old class there were 17 children present but six children were documented as present on the attendance record.

Plan of Correction: We will review attendance policies with our staff and make sure they are updating their roll sheet when children enter the room. If they have a child who is not on their roll then they will add students to their roll for that day.

Standard #: 22VAC40-185-500-B
Description: Based on observation, a nonabsorbent surface was not used for diapering or changing. Evidence: In the Toddler class the diaper changing pad had a large tear down the center, making it absorbent.

Plan of Correction: We have already ordered a replacement changing pad and will begin using it as soon as it arrives.

Standard #: 22VAC40-185-510-A
Description: Based on review of medication documentation, the center did not obtain written authorization from a parent to administer medication. Evidence: There was a medication on site for Child A, but there was no written authorization from the parent to administer the medication.

Plan of Correction: We will have the parent sign the form at pick-up today.

Standard #: 22VAC40-185-510-N
Description: Based on review of medication and documentation, medication with an expired authorization was not sent home within 14 days or disposed of. Evidence: A medication for Child B, with an authorization that expired on 10/12/19, was still on site in the medication box.

Plan of Correction: We will make sure that goes home. The family is on vacation until 1/6/20 so we will return it then.
