Comments:
An unannounced monitoring inspection was conducted on May 29, 2024 from 2:45 pm until 4:28 pm at the center. There were 14 children in care with 11 staff supervising including administrative staff at the facility. The inspector completed observations of the children in care transitioning from napping, eating snack, completing bathroom breaks, handwashing, participating in circle time reading, playing with age appropriate equipment, feedings, and interacting with staff on the carpet. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of 3 children records and 3 staff records were reviewed.

Standard #: 8VAC20-780-130-A
Description: Based on review of children's record and interview, the center did not ensure to obtain documentation of each child immunizations before the child can attend the center.

Evidence:
1. In Child#2's record (start date: 05/28/2024) there was no documentation of immunizations found for review.
2. Staff confirmed the documentation was not available for review during the inspection.

Standard #: 8VAC20-780-160-A
Description: Based on review of staff records and interview, the center did not ensure to obtain documentation of a negative tuberculosis screening for each staff member withing the last 30 calendar days of the date of employment.

Evidence: In Staff#3's record (date of hire: 05/07/24), the tuberculosis screening was dated December 15, 2023.

Plan of Correction: Teacher has gotten updated TB screening/test. An TB screening/test will be w/in 30 days on hire. Director

Standard #: 8VAC20-780-270-A
Description: Based on observation and interview, the center did not ensure all areas and equipment of the center, inside and outside shall be maintained in a clean, safe, and operable condition.

Evidence:

1. On the playground area, there were visible cracks of the wooden fence in two separate areas accessible to the children. There was a visible opening of the fence on the side and the fence ties are missing not connecting the chain links to the top rails in multiple areas.

2. Staff confirmed the current use of playground area by the two's group.
3. Photographs of the playground area observed during the inspection.

Plan of Correction: A ticket was put in to fix the fence. Students will not play in these areas until fixed. Director

Description: Based on observations, the center did not ensure all hazardous substances are kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. In the infant room, there was hand sanitizer on the counter with the labels "keep out of reach of children" and "warning".
