An unannounced renewal inspection was conducted on May 13, 2024, from 9:30am-11:40am. At the time of entrance, 8 children(25pts) were in care with 2 caregivers present. The inspector reviewed 6 children?s records, one household member record, and two caregiver's records. Children were observed during breakfast, circle time, toy play, and engaged in group singing. Areas of non-compliance are identified on the violation notice. Repeat violations were found and are identified within this report.

Standard #: 22.1-289.035-A Description: Based on record review, the family day home did not update criminal history record reports every five years.
Evidence:
1. Caregiver's record did not have an updated criminal history record check. Documentation of the previous criminal history record check was dated 02/05/2019.Plan of Correction: An appointment has made for 05/14/2024 to obtain an updated criminal history record check.
Standard #: 22.1-289.036-A Description: Based on record review, every applicant and household member for the family day home did not obtain a criminal history record check every five years.
Evidence:
1. Caregiver #1's record contained an expired criminal history record check. Documentation of the last criminal history finding was dated 02/05/2019.
2. Household member #1's record contained an expired criminal history record check. Documentation of the last criminal history record check was dated 04/08/2019.Plan of Correction: Appointments have been made for the provider and the household member on 05/14/2024 to obtain an updated criminal history record check.
Standard #: 8VAC20-800-60-B Description: Based on record review, children's records did not contain required information.
Evidence:
1. Child #6's record (Start date-01/02/2024) did not contain a name, address, and telephone number of a second person to contact in case of an emergency if the parent cannot be reached.
2. Child #2's record (Start date-10/03/2022) did nor contain documentation of the review of the child's emergency contact information.Plan of Correction: The provider will ask the parents to provide a second emergency contact and document the record. The provider will also ask the parents to document the record for the annual review.
Standard #: 8VAC20-800-90-C Description: Based on record review, the family day home did not obtain documentation of additional immunizations for a child who is not exempt from the immunization requirements once every six months for children under the age of two years.
Evidence:
1. Child 1's(17months) record (Start date-04/17/2023) did not contain additional immunizations. Documentation of the last immunization update was 04/18/2023.Plan of Correction: The provider will ask the parents to provide an updated immunization record.
