Comments:
An unannounced renewal inspection was conducted on 01/31/2024 from 10:40am to 1:35pm. There were 97 children ages 4 months to age 5 years old supervised by 6 staff. The physical plant, programming, attendance, menus, 5 staff records, 5 children?s records, 7 medications with corresponding authorization records, emergency drills, emergency supplies, and policies were reviewed. Children were observed participating in art, feeding, reading and games. Handwashing procedures were also observed. There was an adequate number of staff present with current certification in Medication Administration Training (MAT), CPR and First Aid, and Daily Health Observation (DHO) training. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22.1-289.035-B-4
Description: Based on review of 5 staff records, the center did not obtain the results of a criminal history record check and a central registry check from all states in which staff members have resided within the last five years prior to employment.

Evidence:
The record for Staff #4 (date of hire 8/1/21) did not contain documentation of a completed central registry record check from all states where they resided within the last five years prior to employment.

Plan of Correction: Ensure all background checks are completed prior to hire.

Standard #: 8VAC20-780-160-A
Description: (Repeat Violation) Based on review of 5 staff records, the center did not obtain documentation of a negative tuberculosis (TB) test or screening for staff at the time of employment and prior to contact with children and within 30 days prior to employment.

Evidence:
1)The file for Staff #1 (date of hire 1/3/24) did not contain documentation of a negative tuberculosis (TB) test or screening prior to employment. The TB screening on file was dated 1/8/24.
2) The file for Staff #2 (date of hire 2/27/23) did not contain documentation of a negative tuberculosis (TB) test or screening prior to employment. The TB screening on file was dated 3/18/23.
3) The file for Staff #3 (date of hire 1/18/22) did not contain documentation of a negative tuberculosis (TB) test or screening prior to employment. The TB screening on file was dated 2/10/22.

Plan of Correction: Ensure all staff members obtain TB test prior to hire.

Standard #: 8VAC20-780-280-B
Description: Based on observation, the center did not ensure that hazardous substances were kept in a locked place using a safe locking method that prevents access by children.

Evidence:
During inspection in the laundry area directly off the hall where the door was unlocked, a container of detergent, a container of bleach and 2 containers of sanitizer were stored on the floor.

Plan of Correction: Store all hazardous substances in locked cabinet off the floor.

Standard #: 8VAC20-780-570-E
Description: (Repeat Violation) Based on observation, the center did not ensure that prepared infant formula was dated and labeled with the child?s name.

Evidence: In the Infant C classroom, a child had a bottle of infant formula that was stored in the refrigerator and was not dated.

Plan of Correction: Director will check bottles each day to ensure they are labeled.
