8VAC20-790 Subsidy Regulations

Comments:

The licensing inspector conducted an unannounced Subsidy Health and Safety inspection on October 17, 2022 from approximately 11:30am until 1:30pm. There were eight children in attendance and a total of two staff members. A total of five children?s files and four staff files were reviewed. A code compliance inspection was conducted on the same date as the SHSI supplemental inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and returned it to me within five business days from today. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). The provider must send documentation to the LI that the background checks have been requested no later than 10 days following this notification dated, October 31, 2022. Description:	Based on review of four staff records and an interview, the vendor did not ensure two staff records contained documentation of all required information. Evidence: 1. The record for Staff #2 did not contain staff's hire date and documentation of completing required training 2. The record for Staff #3 (Date of Employment 09/01/2022) did not contain documentation of completing required training. 3. Staff #4 stated these trainings have been completed but could not locate the documentation during the inspection.

Description:	Based on review of four staff records and interview, the vendor did not obtain, prior to employment, satisfactory results of a fingerprint-based national criminal background check for one staff member. Evidence: 1. The record for Staff #3 (date of employment 09/01/2022) did not contain results of satisfactory results of the fingerprint-based national criminal background check. 2. Staff #4 stated Staff #3 has been working at the center, alone with children since the beginning of September 2022.

Description:	Based on observation, the vendor did not ensure in areas used by children of preschool age or younger that all electrical outlets had protective covers that were of a size that could not be swallowed by children. Evidence: The main classroom where children were observed napping contained two electrical outlet located on the walls that did not not contain protective covers.
