Comments:
An unannounced monitoring inspection was conducted Monday, October 21, 2019 to determine the center's compliance with religious exempt requirements. The inspection was initiated at approximately 1:20 PM and concluded at approximately 4:30 PM. A total of 58 children were present during the inspection and the following staff to child ratios were observed:
Twos: 2 children and 1 staff
Threes: 9 children and 1 staff
Fours: 8 children with 1 staff
Pre-K: 13 children and 2 staff
Kindergarten ? 2nd Grade: 11 children and 1 staff
3rd ? 5th Grade: 15 children with 1 staff

Children were observed napping, washing hands and taking bathroom breaks, eating snack, reviewing colors in English and Spanish, playing with toys, looking at books and completing homework. Ten children?s records and 10 staff records were reviewed. All areas of the center were inspected including the classrooms, hallways, bathrooms and outdoor play areas. Policies and procedures were discussed and information disclosed to parents, guardians and public was reviewed. Violations were cited during the inspection. Violations:
Standard #: 22VAC40-191-40-D-2
Description: Based on a review of 10 staff records and interview, the center did not ensure that one staff record contained satisfactory results of the Child Protective Services Central Registry check within 30 days of the individual beginning employment or service.
Evidence:
The record for Staff #3 (hire date 02/02/2019) did not contain satisfactory results of the Child Protective Services Central Registry. Staff #1 confirmed this information while the licensing inspector was on site.

Plan of Correction: A copy of the form was documented as being sent in 02/2019. I will contact the agency and check if the search was completed. If not, I will resend.

Standard #: 63.2(17)-1716-A
Description: Based on interview, documentation review and inspection of the facility, the center did not disclose in writing to the parents or guardians of the children in the center the fact that it is exempt from licensure and the qualifications of the personnel employed.
Evidence:
The documents reviewed that were posted in the facility and distributed to the parents and guardians did not contain information regarding the center being exempt from licensure and qualifications of personnel. Staff #1 stated she was not aware of this being a requirement.

Plan of Correction: I will complete the model form that is sent to me by the licensing inspector and hand it out to parents and include it in the enrollment packets.

Standard #: 63.2(17)-1716-A-6
Description: Based on review of documentation and interview, the center did not describe in written statement to parents or guardians and made available to general public aspects regarding the center's operations concerning physical facilities, enrollment capacity, food services, health requirements for staff and public liability insurance.

Evidence:
Documentation that is given to parents and guardians and made available to the public was reviewed. The information reviewed did not contain information describing physical facilities, enrollment capacity, food services, health requirements for staff and public liability insurance. Staff #1 stated she was not aware it was a requirement to have this information available in writing.

Plan of Correction: I will complete the model form that is sent to me by the licensing inspector and hand it out to parents and include it in the enrollment packets.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on a review of 10 staff records and interview, the center did not ensure that two staff records contained satisfactory results of the child abuse and neglect registry from any other state in which the individual has resided in the preceding five years.

Evidence:
The records for Staff #2 (hire date 8/21/19) and Staff #9 (hire date 9/6/19) indicated the staff have lived in another state in the last five years. Staff #1 stated a child abuse and neglect registry has not been requested from the other states.

Plan of Correction: I will start the process to request the child abuse and neglect search from the other states and provide a copy of the request to the licensing inspector.
