Comments:
A code compliance inspection was conducted September 21, 2023. The licensing inspector was onsite from approximately 9:45am to 12:45pm. There were 16 children (two to five years of age) with four staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and medication. A total of five children's records and five staff records were reviewed.


Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.031-A-3-f
Description: Based on interviews, the center did not ensure during designated daily rest periods for children ages 16 months to six years, one staff member was present with the children under supervision. In such cases, at least one staff member needed to be always physically present in the same space as the children under supervision.

Evidence:
Staff #2 stated occasionally a staff member would monitor and walk between two separate classrooms when the center was short staffed. Children would be asleep in both classrooms.

Plan of Correction: We will sleep the children in the same room until additional staff are hired.

Standard #: 22.1-289.031-B-5
Description: Based on observation and interview, the center failed to implement established procedures that ensured all areas of the premises accessible to children were free of obvious injury hazards.
Evidence:
1. Several areas of exposed landscaping fabric were observed on the children?s playground. The exposed fabric created a tripping hazard.
2. Unlocked cleaning chemicals were in the Preschool Classroom and the director?s office. Both areas had children in care. The cleaning chemicals were accessible to the children.
3. A member of management was shown the hazards during the inspection. The member of management confirmed these observations during the inspection.

Plan of Correction: I will have the maintenance team remove the exposed fabric from the playground.

Standard #: 22.1-289.035-A
Description: Based on review of five staff records and interview, the center did not ensure one employee completed background check at least every five years for staff or volunteer.

Evidence:
1. The record for Staff #2 (first day of employment 10/27/2017) contained a fingerprint based national criminal record check dated 08/22/2028.
2. Staff #2 confirmed as of 09/21/2023 an appointment to obtain an updated background check had not been completed.

Plan of Correction: An appointment for the updated background check was scheduled while the licensing inspectors were on site.
