A Code compliance monitoring inspection was initiated on 11/3/2022 and concluded on 11/3/2022. There were three children present with one staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and one staff record was reviewed. The inspection started at 9:30 am and concluded at 10:45 am.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22.1-289.031-A Description: Based on observations and interviews the center failed to post the fact that it is exempt from licensure in a visible location on the premises, the qualifications of the personnel employed therein.
Evidence:
During the inspection on 11/3/2022 it was observed that there are no postings at the center disclosing informing the parents that it is exempt from licensure and the qualifications of the personnel employed therein.Plan of Correction: The principle posted the letter of exemption and the public disclosure statement at the preschool room.
Standard #: 22.1-289.031-B-4 Description: Based on record review and interviews the center failed to implement their procedure to ensure that all children are in compliance with immunization provisions.
Evidence:
Two of the three children's records reviewed did not have an immunization report on file.Plan of Correction: The principal contacted parents and requested that immunizations be provided.
