Comments:
An unannounced monitoring inspection was conducted on 06/04/2024 from 10:50 a.m. to 2:45 p.m. There were 129 children present, ranging in four months to five years old with 27 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 10 child records and 10 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-130-E
Description: Based on review of records, the center failed to obtain documentation of additional immunization once every six months for children under the age of two years old.
Evidence:
1. Child #1?s date of birth is 12/11/2022 and start date was 07/31/2023. Immunizations on file were dated 07/31/2023 and 03/02/2024. Immunizations had not been obtained every six months.
2. Child #2?s date of birth is 12/15/2022 and the start date was 07/10/2023. Immunizations on file were dated 06/19/2023. Immunizations had not been obtained every six months.

Plan of Correction: Family was immediately provided guidance on updating forms.

Standard #: 8VAC20-780-60-A
Description: Based on review of documentation, the center failed to maintain a record for each child that contained the required information.
Evidence:
1. Child #4?s record was missing an annual update, one emergency contact and proof of identity.

Plan of Correction: Family was immediately provided guidance on updating forms.

Standard #: 8VAC20-780-510-F
Description: Based on review of documentation, the center failed to ensure that the medication authorization form is available to staff during the entire time it is effective.
Evidence:
1. Child #6, #11, #12 and #13 had emergency medication on site but there was no medication authorization forms.
2. Staff #14 confirmed that the medication for Child #6, #11, #12 and #13 were effective.

Plan of Correction: While the center has had properly completed allergy care plan forms that that included medication authorization, the separate forms had not been completed by the families/center. Forms were immediately given to the families and asked to omplete by 06/21/2024

Standard #: 8VAC20-780-550-G
Description: Based on review of documentation and interview, the center failed to maintain documentation of shelter-in-place drills.
Evidence:
1. There was no documentation of shelter-in-place drills at the time of the inspection. Staff #14 confirmed that there was no documentation of the shelter-in-place drills.

Plan of Correction: SIP drills were documented incorrectly as lockdown drills. Staff has now been retrained and SIP drill is scheduled to be completed on 6/25/2024.
