Technical Assistance:
Consulted with Director and recommended participating in Phase I and II training to better understand and stay up to date on standards.
Effective January 1, 2025, the VDOE will begin determining compliance with ? 22.1-289.057 of the Code of Virginia, which is legislation passed by the General Assembly in 2020. The law requires all licensed child day programs, religious exempt child day centers that serve preschool age children, and certified preschools to test potable drinking water. The law requires that programs submit their plans and test results to the Virginia Department of Health Office of Drinking Water (VDH ODW) and the Superintendent. If the results of the testing indicate elevated lead levels, the program shall remediate, retest, and resubmit results to VDH ODW and the Superintendent. There is an additional alternative bottled water option that comes with additional requirements. Comments:
An unannounced monitoring inspection was conducted 5/22/2024 from approximately 9:40am-12:10pm. There were 22 children directly supervised by 5 staff in 4 classrooms, all in ratio. The physical plant, 2 staff records, 3 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group lessons, practicing numbers, listening to a story, washing hands, and eating snack. There was an assortment of age appropriate materials for the children in care. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22.1-289.035-B-2
Description: Based on a review of staff records, the center did not ensure Federal Bureau of Investigation for the purpose of obtaining national criminal history record information was completed.
Evidence: Staff #2's record did not have FBI/OBI clearance that fingerprinting was done.

Plan of Correction: Director to contact staff and HR to complete the clearance process.

Standard #: 22.1-289.035-B-4
Description: REPEAT
Based on a review of staff records, the Director did not ensure central registry and sex offender registry check were on record for any state in which the individual has resided in the preceding five years.
Evidence: Staff #2 did not have out of state central registry or out of state sex offender registry checks on file during the inspection.

Plan of Correction: Director to contact HR and staff to complete clearance.

Standard #: 8VAC20-780-80-A
Description: Based on record review, for each group of children, the center did not maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurred.
Evidence: Attendance record in the Starfish room did not show any children in attendance during the inspection, there were 5 children in the classroom.

Plan of Correction: Staff marked attendance during inspection. Director will periodically check to review attendance record daily and to include time of arrival and departure in each classroom, by updating the form.

Description: Based on observation and record review The center did not obtain written parent authorization noting any known adverse reactions to sunscreen identified for a child's use.
Evidence: Child #1 authorization for sunscreen in child's classroom, identified for their use expired 6/8/2023.

Plan of Correction: Director to request updated authorization for sunscreen.

Description: Emergency drill documentation did not contain all required information.
Evidence: Emergency drills did not contain the number of staff and children participating in the drill.

Plan of Correction: Director creating new form to include all required information on drill record.

Standard #: 8VAC20-780-560-G
Description: Based on observation, container with food brought from home did not have required information.
Evidence: 22 lunch bags brought from home 6 did not have date and 3 had incorrect dates.

Plan of Correction: Director will request parents check for accurate information on lunch bags during drop off.

Description: Based on observation and interview, documents related to the terms of the license that are required to be posted on the premises were not posted.
Evidence: Current inspection were not not posted or available during this inspection.
