Standard #: 22VAC40-111-100-A Description: Based on record review, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1) The records for Child #1 (start date 08/16/2021) and Child #2 (start date 09/18/2020) did not contain the results of a physical examination.Plan of Correction: Will contact the parents to request the physical examination. Child #1 has an appointment on 10/15 for the child's annual review.
Standard #: 22VAC40-111-60-B Description: Based on record review, children's records did not contain all required information.
Evidence:
1) The record for Child #1 did not contain the name, address and telephone number of a second person to contact in case of an emergency if the parent cannot be reached.Plan of Correction: Will contact the parents to request the missing information.
