Comments:
A subsidy health and safety (SHSI) inspection was completed on 1/31/2024 from 12:00 pm - 4:05 pm. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined violations with applicable standards or law. Violations are identified on the violation notice.

Violations:
Standard #: 8VAC20-790-600-C
Description: Based on a review of records and interview, the subsidy vendor did not always ensure that orientation training for staff is completed on the required facility -specific topics prior to the staff member working alone with children and within seven days of the date of employment or the date of subsidy vendor approval.
Evidence:
Ten staff records were reviewed.
The record for staff #3, whose date of employment was documented as 12/15/2023, did not contain documentation of the orientation training on the required facility -specific topics.
This staff has been employed for more than 7 days.

Plan of Correction: The program director stated that staff #3 had completed orientation at the time of hire, but that the training documentation could not be located.
The orientation will be reviewed with staff #3 and documented as completed within 10 days.

Standard #: 8VAC20-790-600-F
Description: Based on a review of records and interview, the subsidy vendor did not always ensure that staff who work directly with children annually complete the department's health and safety update course.
Evidence:
Ten staff records were reviewed.
The record for staff #7 ,whose date of employment was documented as 10/31/2022 completed the last annual health and safety update training on 10/30/2022, did not contain documentation of an updated annual health and safety update training for 2023.
The program director confirmed that staff #3 had not yet completed this training.

Plan of Correction: The program administrator stated that she has reminded staff #7 that this training must be completed. Staff #7 will complete the training and submit the certificate for her file within 10 days.
