An unannounced Monitor Inspection was conducted on April 29, 2024. There were 43 children, ages 5 wks to 5 years old, 12 staff and 2 administrative staff present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, care of children, preventing the spread of disease, emergencies and nutrition. Medications for children were reviewed during the inspection. Licensing Inspector review 4 children's records and 5 staff records. The children were observed during circle time, center time and active time on the playground. The inspector arrived for the inspection at 12:30 PM and departed the center at 3:55 PM.
Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-270-A Description: Based on inspection, the Center failed to maintain areas and equipment of the center, inside and outside, in a clean, safe and operable condition.
EVIDENCE:
1. Ceramic tiled floors in multiple classrooms were excessively stained with dirt.
2. Cinder blocked walls in multiple classrooms were stained with dirt; paint was worn and dingy
3. Unlocked storage cabinet underneath the sink in room 106 contained a hazardardous sticky mouse trap that was accessible to the children in care.
4. Bookcase in room 102 had splintered wood surface and edges.
5. Window blind cords in room 106 were accessible to the children in care causing a strangulation hazard.
6. Foam border on 3 toy shelves in the infant classroom were torn; pieces of the foam hazardous to the infant in care.Plan of Correction: Not available online. Contact Inspector for more information.
