An unannounced, on-site monitoring inspection was initiated on 6/24/2024 and the inspection concluded on 6/24/2024. The on-site inspection began at 2:30 pm and ended at 3:15 pm. The inspector reviewed compliance in the areas listed above. The inspector reviewed one child record, the provider's record and three household member records. The inspection included document review and tour of the home.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-800-830-C Description: Based on record review, the provider did not document the completion of emergency evacuation drills.
Evidence: There was no documentation recorded from February, 2024 to June, 2024 of a completed emergency evacuation drill.Plan of Correction: Provider will conduct and document emergency evacuation drills monthly.
Standard #: 8VAC20-800-180-A Description: Based on record review, three adult household members did not have an updated tuberculosis screening on file.
Evidence: HHM #1, HHM #2 and HHM #3 did not have an updated tuberculosis screening documented in the file.Plan of Correction: Provider will obtain updated TB screening on all household members.
Standard #: 8VAC20-800-460-C Description: REPEAT VIOLATION
Based on record review, the provider failed to have a pet vaccinated for rabies.
Evidence: Pet #1, Pet #2 and Pet #3 did not have an updated rabies vaccine. Vaccine for these animals expired 4/10/2024.Plan of Correction: Provider will ensure pets in the home have update rabies vaccine.
