A complaint investigation was initiated on 3/25/2024 and completed on 5/22/2024.

Comments:
A complaint inspection was conducted on 4/10/2024 in response to a complaint received on 3/22/2024 regarding age-appropriate food services. Interviews were conducted and menus and records were reviewed. The information gathered during the investigation supports the allegation, so the complaint is determined to be "valid". Violations are listed on the attached notice and were reviewed with the owner during the exit interview. If you have any questions, contact the licensing inspector at (757)404-2322.

Description: Based on interviews and records review, it was determined the facility did not ensure that when food is brought from home, provisions were made to have or obtain appropriate snacks or meals if a parent forgets to bring food to the center.
Evidence:
1.The parents of a 5-month-old provided food from home. The parents also provided a weekly menu of the food the infant would be eating each day. The infant was eating stage 1 food.
2. On 3/21/2024 the parent forgot to bring the baby food and stated the infant should be served the food listed on her weekly menu, apples, and oatmeal.
3.The center did not have or obtain apples and oatmeal and instead substituted a Gerber Stage 2 meal containing banana, carrot, mango, and oatmeal.
4.The manufacturer?s label on the Gerber Stage 2 packaging indicated the food was appropriate for infants aged 6-8 months old. The infant was 5 months old.
5.The center did not contact the parent prior to serving the food to ask if a substitute could be given.
6.There was no written or verbal permission to introduce stage 2 foods to the infant.
7.The parent picked the infant up approximately 30 minutes after the infant was fed. The parent noticed redness and swelling and took the infant to the doctor for treatment for an allergic reaction.
