Staff #3's (Date of Hire: 7/23/19) record does not contain documentation that two references were checked as to character, reputation, and competency before beginning employment. Staff #3's record contained documentation of 1 completed reference check. Plan of Correction: The 2nd employee reference was obtained 08/14/19.
