Comments:

An in person subsidy health and safety inspection was conducted on October 24, 2022. There was one child in care with the vendor. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice and discussed with the center representative during the exit interview.

Description:	Based on record review and interview the vendor failed to ensure that each household member submits to fingerprinting. Evidence: 1 - Household Member #2 turned 18 years old in September 2022, however there is not a fingerprint background check on file or the household member. 2 - The vendor confirmed that there was not fingerprint background check on file for Household Member #2.
