Comments:
An unannounced monitoring inspection was conducted on August 8, 2022 from approximately 9:52 am to 11:05 am. There were 42 children in attendance and a total of 10 staff present. 5 staff records and 5 children?s records were reviewed. All areas of the center were observed including classrooms, hallways, and bathrooms. The children were observed listening to music, playing outside, and playing with toys, while the licensing inspector was on the premises.

Emergency drills and fire inspections were in compliance. The most recent Health Inspection was on 07/26/22. The most recent Fire Inspection was on 07/25/22. The most recent fire drill was on 07/18/22. Shelter in Place Drills were conducted twice with the most recent on 03/09/22. The First Aid Kit had all components. The license, current inspection, and evacuation plan were posted and emergency numbers were posted by each phone.

Violations were cited as a result of this inspection.

Standard #: 8VAC20-780-270-A
Description: Based on observation, the center did not ensure that areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: 1) There were several spots on the wall in the Toddler classroom with peeling paint. 2) The floor mats in the toddler room were peeling and had exposed foam. 3) The sink in the preschool classroom was pulling away from the wall. 4) The fence on the playground had several areas where the chain link was not attached to the posts as well as broken wire sticking out

Plan of Correction: Per the Director "1) the wall behind the sink in the toddler room was spackled with fast dry mud, sanded down and repainted and also applied a new life time guaranteed silicone around entire sink. 2) Mats were thrown away. 3) Sink set up in upstairs bathroom with metal and affixed to wall. Repainted and lifetime silicone applied. 4) Fence on playground was fixed on top and bottom with chain links fence ties. "

Description: Based on observation, and interview, the center did not ensure that the staff to children ratios were followed at all times. Evidence: At approximately 10:30 am Classroom #2 had 8 children ages 16 months to 2 years with 1 teacher. The second teacher walked back into the classroom at approximately 10:32 am. When asked, the teacher stated she had walked down the hall to check the laundry. The ratio for children this age is 1:5.

Plan of Correction: Per the Director "before leaving the room the teacher will call for someone to come in the room while she steps out, that way the staff to child ratio is always met."
