Evidence: The menu posted during the inspection was dated for the previous week.Plan of Correction: We'll correct the date.
Standard #: 8VAC20-800-690-D Description: Based on observation, the diaper changing surface does not meet all requirements.
Evidence: The diaper changing surface has several tears and was not non-absorbent.Plan of Correction: I have a new mat that I need to replace it with.
