An unannounced monitoring visit and facility tour was conducted on 05/23/2024. At the time of inspectors' arrival there were 27 preschool aged children in care with 5 staff members and 2 administrators. Children were observed interacting with staff, nap, and hand washing. A sample of 5 children's records and 5 staff records were reviewed. Injury reports and medications were also reviewed. Assistant Director reported that there have been no injuries and medications are not being administered at this time. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.

Standard #: 8VAC20-780-270-A Description: Based on observation it was determined the center did not ensure that all areas and equipment of the center was maintained in a clean, safe and operable condition.
Evidence:
1. Outside in the play area there was:
-Broken plastic storage bin
-Splintered, rotting wood on the free standing garden planter
-Torn basketball net
2. In the preschool classrooms there were freestanding, unanchored play kitchen equipment that rocked when pulled on, presenting a potential tipping hazard.Plan of Correction: Not available online. Contact Inspector for more information.
