Technical Assistance:
The facility's HVAC system was not operating. Maintenance personnel onsite. Each occupied classroom had portable cooling units to maintain the temperature of each classroom at a comfortable level.

Effective January 1, 2025, the VDOE will begin determining compliance with ? 22.1-289.057 of the Code of Virginia, which is legislation passed by the General Assembly in 2020. The law requires all licensed child day programs, religious exempt child day centers that serve preschool age children, and certified preschools to test potable drinking water. The law requires that programs submit their plans and test results to the Virginia Department of Health Office of Drinking Water (VDH ODW) and the Superintendent. If the results of the testing indicate elevated lead levels, the program shall remediate, retest, and resubmit results to VDH ODW and the Superintendent. There is an additional alternative bottled water option that comes with additional requirements. Comments:
An unannounced, on-site monitoring inspection was initiated and completed on 07/17/2024, as a part of the licensure period. The on-site inspection began at 11am and ended at 1:30pm. The inspector reviewed compliance in the areas listed above. There were 48 children present and 8 staff. The inspector reviewed 4 children?s records and 3 new staff records on-site This inspection included document review such as injury logs, policies and procedures, emergency drill logs, medication administration authorization forms, tour of the facility, staff interviews and staff and child observations.

Violations:
Standard #: 8VAC20-780-60-A
Description: Based on child record review, one record was incomplete.
Evidence:
Child #1's record did not include 2 emergency contact names, addresses and phone numbers.

Plan of Correction: Emergency contacts will be obtained from the child's parents.

Standard #: 8VAC20-780-420-E-3
Description: Based on child record review, staff did not confirm on an annual basis with parents that the required information in the child's record is up to date.
Evidence:
Staff did not confirmannually with Child #4's parent that the information in the record was up to date.

Plan of Correction: This file will be reviewed by the parent. All child files will be reviewed annually.

Description: Based on observation of one classroom, hand washing was not completed.
Evidence:
The hands of 3 children were not washed in the Toddler classroom prior to being served lunch.

Plan of Correction: Handwashing was discussed with this staff member. Observation of hand washing will be made.

Description: Based on review of the first aid kit taken on field trips, the first aid kit did not have all required items.
Evidence:
The first aid kit that was taken on a field trip today did not include scissors, tweezers, antiseptic cleaner or a thermometer.

Plan of Correction: The missing items will be placed in the first aid kit before the next field trip.

Standard #: 8VAC20-780-560-G
Description: Based on observation of food brought from home, food was not properly labeled.
Evidence:
Two lunch boxes located in the PreK 2 classroom was not labeled with today's date.

Plan of Correction: Staff will label all food with the child's name and date if the parents did not.
