An unannounced, on-site renewal inspection was initiated on 6/20/2024 and the inspection concluded on 6/20/2024. The on-site inspection began at 10:20AM and ended at 12:16PM. The inspector reviewed compliance in the areas listed above. There were 15 children and 4 staff present. The inspector reviewed 3 children?s records, 1 staff record and 2 agent backgrounds on-site on 6/20/2024. This inspection included document review, tour of the facility and observations.
Discussion on standards related to resilient surfacing.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 6/26/2024. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-40-E Description: The operational responsibilities of the licensee shall include ensuring the the center?s activities, services and facilities are maintained in compliance with these standards and the terms of the current license issued by the Department.
Evidence: The current license issued by the department has a stipulation of no more than 5 children under the age of 2 and a half. There were 8 children under the age of 2 and a half present during the inspection.Plan of Correction: Provider will ensure adherence to stipulation on license per building officials.
Standard #: 8VAC20-780-60-A Description: Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the name, address and phone number of two designated people to call in an emergency if a parent cannot be reached.
Evidence: The record for Child #1 did not have a second emergency contact listed. The start date for Child #1 was 6/20/2024.Plan of Correction: Provider will ensure a second emergency contact is listed.
